Prochlorperazine Maleate

Indications

• Radiation therapy, cancer chemotherapy, surgery, and other illnesses can induce severe nausea and vomiting. Prochlorperazine Maleate can help.
• Meniere's disease and kindred inner ear problems include nausea, vomiting, and attacks of dizziness or spinning feelings (vertigo).
• For the treatment of psychotic disorders including schizophrenia (hallucinations and hostility).
• Mania acute.
• For the treatment of generalized non-psychotic anxiety in the short term.

Pharmacology

Dopamine and histamine antagonist prochlorperazine. The blocking of histamine H and dopamine D2 neurotransmitter receptors in the chemoreceptor trigger zone and vomiting centre is the mechanism of antiemetic activity. It also has a modest anticholinergic action and helps to prevent acid reflux by raising the tone of the lower esophageal sphincter.

Dosage & Administration

Antiemetic-

Children (not recommended for children under the age of 10 kg or two years):

• 5 mg every 12-24 hours as needed for 10-14 kg; maximum: 7.5 mg per day
• 15 to 18 kg: 2.5 mg every 8 to 12 hours as needed; maximum: 10 mg per day
• 5 mg every 8 hours or 5 mg every 12 hours as needed for 19-39 kg; maximum: 15 mg per day.

Adults: 5-10 mg three to four times per day; normal maximum: 40 mg per day

Antipsychotic-

Children (not recommended for children under the age of 10 kg or two years):

• 5 mg 2-3 times per day for children aged 2 to 12 years.
• Increase dosage as needed to a maximum daily dose of 20 mg for children aged 2 to 5, and 25 mg for children aged 6 to 12.

Adults: 5-10 mg 3-4 times/day; for severe disorders, dosages up to 150 mg/day may be required in some patients.

Nonpsychotic anxiety-

• Adults: Take 15-20 mg per day in divided doses; do not take more than 20 mg per day or for more than 12 weeks.
• Elderly: Start with 2.5-5 mg 1-2 times/day; gradually increase dose by 2.5-5 mg/day every 4 to 7 days; increase dosing intervals (twice daily, thrice daily, etc) as needed to control response or side effects; maximum daily dose should probably not exceed 75 mg in the elderly; gradual increases (titration) may prevent or reduce the severity of some side effects.

Prochlorperazine can be taken regardless of what you're eating.

Interactions

Alcohol, barbiturates, and other sedatives may have a more depressing effect on the CNS. Antacids, antiparkinson's medications, and lithium may interfere with Prochlorperazine absorption. Propanolol and phenobarbital plasma concentrations may be affected by this medication.

Contraindications

Hypersensitivity to prochlorperazine or any other component of the formulation, severe CNS depression, coma; not to be used in children under the age of two years or weighing less than ten kilograms.

Side-Effects

Drowsiness; muscle spasms in the jaw, neck, and back; fine worm-like tongue motions; rhythmic face, mouth, or jaw movements; sluggish or difficult speech; difficulty swallowing; tremors; shuffling walk; skin rash; yellowing of the skin or eyes.

Pregnancy

There has been no proof of this drug's side effects during pregnancy or lactation.

Precautions

When conducting duties that need awareness, such as driving or operating machinery, exercise caution. Alcohol consumption might result in excessive sleepiness. This drug may make you more sensitive to the sun. Sun exposure should be avoided for long periods of time, and sunscreen and protective clothes should be worn whenever someone is exposed to the sun. As a result of the medication's ability to suppress perspiration, it makes it more susceptible to heatstroke.
